Were stakeholders kept up to date on post production related actions?    
As you apply quick wins related to usability and performance, make sure you inform the customer on what is being done. If required discuss their priority beforehand with them. The same is valid to warranty activities.

Did you capture if the customer expectations were met and the reasons for it?

Make sure you sent to the representatives of IT and to the relevant business users directly involved in the project (Business Users, Project Manager and IT), the customer survey and analyse its results sharing them with the customer and the team. If relevant, discuss it with your customer.

Are stakeholders aware of the project closure?    
Project closure must be be communicated. Prior to perform this communication, ensure that all expectations were managed and the customer will not be surprised with it.

Did you share lessons learned and project metrics?

Make sure you conduct an internal retrospective meeting with your senior manager, DM, EM and involved salesperson, where you cover lessons learned, project issues, financials, etc. Also make sure your record your project in CIS and after send an internal e-mail informing of your project success (see an example here).

Was the plan you defined for the application continuity successfully implemented?    
Your participation in the project ended but the application life cycle continues. To it’s success you did handovers session and proposed initiatives for business continuity and/or customer autonomy. In the end of the post-production evaluate if they are in place and the application life cycle is not compromised.

Make sure you deliver the Project Hand-over document that gathers relevant project information to ensure an autonomous, offline and asynchronous handover.

You must do your self-assessment and give your feedback on your team members performance. Invite your team to do the same.

Did you monitor and address feedback to identify usability quick wins?    
Frequently some usability issues only come up when users start using the systems to do real work. Make sure you create the opportunity to go to the the field and see final users using the system, gather feedback, identify and implement any quick wins that fit within the remaining budget.

Did you monitor and address the defects and potential performance issues detected after go-live?    
Frequently some defects and performance issues only come up when users start using the systems to do real work. Make sure you keep track of any defects reported and address those deemed critical. Also monitor the system and promote proactive performance tuning.
